DIAGNOSIS
SUBJECTIVE: “eto nga at hianng hina siya, hindi na niya magalaw ang right side niya” as stated by the significant other
IMPAIRED PHYSICAL MOBILITY rt neuromuscular involvement (right sided paresthesia) aeb inability to purposefully move body parts.
OBJECTIVE: -grade of 0 in the level of functioning -paresthesia in the right side of the body -0/5 muscle strength in the RUE -1/5 muscle strength in the RLE -impaired coordination -inability to purposefully move body part

PLANNING DISCHARGE PLAN: After 2 week of Nursing Intervention the client will be able to move and do minimal task such as: -go to the toilet with minimal assistance -ambulate moderately -eat without assistance -do tooth brushing without assistance
INTERVENTION
RATIONALE
*Observe movement when client is uaware of observation
*to note any incongruencies w/ reports or abilities
*schedule activities with adequate rest periods during the day
*to reduce fatigue and increase comfort.
