FISIOLOGI JANTUNG FETUS Semua keperluan fetus dan pembuangan nya melalui plasenta dan pembuluh umbilicus. Plasenta berfungsi sebagai paru-paru dan traktus digestivus. Pada janin, darah dengan oksigen relative cukup mengalir dari plasenta melalui vena umbilicus. Separuh jumlah darah ini mengalir melalui hati, sedang sisanya memintas hati melalui duktus venosus ke vena cava inferior, yang juga menerim darah dari hati (melalui vena heoatika) serta tubuh bagian bawah. Sebagian besar darah dari vena cava inferior, mengalir ke dalam atrium kiri melalui foramen ovale, selanjutnya ventrikel kiri, aorta desenden dan sirkulasi koroner  mendapat darah dengan oksigen yang cukup. Sebagian kecil darah dari vena cava inferior memasuki atrium kanan , lalu ventrikel kanan melalui katup trikuspidalis. Darah yang kembali dari leher dan kepala janain memasuki atrium kanan melalui vena kava superior dan bergabung dengan darah dari sinus koronarius menuju ventrikel kanan selanjutnya ke arteri pulmonalis. Pada janin hanya 15% darah dari ventrikel kanan yang memasuki paru, selebihnya melewati duktus arteriosus menuju ke aorta desenden, bercampur dengan darah dari aorta asenden. Darah dengan kandungan oksigen rendah ini akan mengalir ke organ-organ tubuh , dan juga ke plasenta melalui arteri umbilicus yang keluar dari arteri iliaka interna. Pada janin normal, ventirkel kanan memompakan 60% curah jantung sisanya dipompa kan oleh ventrikel kiri. Curah jantung janin di distribusikan sebagai berikut : 40%  aorta asenden 4%  sirkulasi koroner 20%  arteri leher dan kepala 16%  aorta desenden 60% dipompakan ke arteri pulmonalis, tetapi hanya 8% menuju paru, dan 52% melewati duktus asrteriosus menuju aorta desenden. PERBEDAAN SIRKULASI JANIN DAN KEADAAN PASCA LAHIR Pada janin terdapat pirau intrakardiak (foramen ovale ) dan ekstrakardiak ( duktus arteriosus Botalli dan duktus venosus Arantii) . Arah pirau dari kanan ke kiri, atrium kanan ke atrium kiri melalui foramen ovale dan dari a.pulmonalis ke aorta melalui duktus arteriosus. Pada pasca lahir pirau tersebut normalnya tidak ada. Pada janin, ventrikel kanan dan kiri bekerja serentak, sedangkan pasca lahir ventrikel kiri bekerja lebih awal.

Pada janin ventrikel kanan memompa darah ke tempat tahanan yang lebih tinggi, yaitu tahanan sistemik, sedangkan ventrikel kiri melawan tahanan rendah yaitu plasenta. Pasca lahir terjadi sebaliknya. Pada janin darah yang dipompa oleh ventrikel kanan sebagian besar menuju ke aorta melalui duktus arteriousu, dan hanya sedikit yang menuju paru.. Pasca lahir, seluruh darah dari ventrikel kanan menuju paru. Pada janin, paru memperoleh oksigen dari darah yang diserap melalui plasenta, pasca lahir paru member oksigen kepada darah. Pada janin, plasenta merupakan tempat utama pertukaran gas, makanan dan eksresi. Pasca lahir, system organ sudah mengambil alih hal tersebut.

PERUBAHAN SIRKULASI NORMAL SETELAH LAHIR Perubahan-perubahan yang terjadi : Tahanan vascular pulmonal turun dan aliran darah nya meningkat Tahanan vascular systemic meningkat Duktus arteriosus menutup Foramen ovale tertutup Duktus venosus tertutup Penurunan tahanan paru terjadi akibat ekpansi mekanik paru, peningkatan saturasi oksigen arteri pulmonalis dan tekanan oksigen alveolar. Penutupan Duktus Arteriosus Faktor yang berperan : Peningkatan tekanan oksigen arteri menyebabkan konstriksi duktus asrteriosus Peningkatan kadar katekolamin  konstriksi duktus Penurunan kadar prostaglandin  konstriksi duktus Paru mengembang – penurunan resitensi – darah mengalir ke paru – tekanan aorta tinggi – duktus arteriosus tidak menerima darah  menutup. Penutupan Foramen Ovale Faktor yang berperan : Konstriksi duktus venosus, darah tidak mengalir  penutupan foramen ovale

Resistensi pulmo menurun  darah masuk melalui paru, tekanan paru lebih tinggi Penurunan tekanan atrium kanan Penutupan duktus Venosus  terjadi karena pemotongan tali pusat – terjadi trauma – konstriksi pembuluh darah – penurunan tekanan – duktus venosus tidak menerima darah  penutupan duktus venosus.

In the normal fetus , right ventricel pump cardiac output remaining 60 % is pumped out by the left ventricle . Fetal cardiac output is distributed as follows : - 40 %  ascending aorta - 4 %  coronary circulation - 16 %  descending aorta 60 % is pumped into the pulmonary artery , but only 8 % to the lungs , and 52 % through the duct asrteriosus towards descending aorta . FETAL CIRCULATION DIFFERENCES AND CIRCUMSTANCES AFTER BIRTH
