ADULT ASIAN MALE SUFFERED MULTIPLE GUNSHOT WOUNDS: 1. Gunshot wound A, close range penetrating gunshot wound, posterior head, right side. A. The bullet causes patchy subscalpular hemorrhage, patchy skull fractures, patchy subdural hemorrhage, patchy subarachnoid hemorrhage, extensive damage to the brain tissue, and a distorted, copper jacketed and medium caliber bullet is lodged and recovered in the frontal lobe. B. There is a zone of soot around the entrance wound. C. Direction of the wound track: posteroanterior and slightly upward. 2. Gunshot wound B, close range penetrating gunshot wound, posterior cervical spine. A. The bullet causes extensive damage to the cervical spine and to the bases of the brain tissue, and a distorted, copper jacketed and medium caliber bullet is lodged and recovered in the sinus. B. I hereby certify that I, Caroline Comajig M.D., J.D., have performed an autopsy on the body of Sgt.___________________on the ___day of _____ at _____ in the Cebu Medical Examiner’s Office. The
purpose of this report is to provide a certified opinion to the case at hand. The facts and findings to these conclusions are filed with the Department of Health.
EXTERNAL EXAMINATION The body is that of an adult asian male measuring __________ centimeters and weighing ______ pounds.The body is normally developed and appears consistent with the given age of 37 years. He is received with blue police polo uniform, white undershirt, blue police uniform pants, black belt, white briefs, two black socks and two black leather shoes. The blue police polo is stained with blood. Post mortem changes consist of fixed posterior livor mortis and full rigor mortis. The scalp hair is black, straight and short. The corneas are cloudy. The sclera and conjunctivae are unremarkable. No discrete petechiae are present. The irides are brown. The mouth contains natural teeth. The palate and frenula are intact. The nose, ears and mouth show no abnormalities. The neck is of normal configuration except in the area where projectiles made entry and there are no palpable masses. The thorax is symmetrical. The abdomen is flat. The axillae, external genitalia and anus are without trauma. The extremities are symmetrical and normally developed. There are no evidence of emergency medical intervention. EVIDENCE OF INJURY: At the time of the autopsy, there are a total of two gunshot wounds to the posterior head. The sequence of the infliction cannot be determined for certain and is arbitrarily labeled in the report. GUNSHOT WOUND A: This is a 3/16 inch close range penetrating gunshot wound on the back of the head, at 5 inches below the top of the head and two inches to the right of the posterior midline. There is a zone of soot around the entrance wound which measures 2 inches x 1 inch in greatest dimension. A hemorrhagic wound track es posteroanterior and slightly upward causing patchy subscalpular hemorrhage, patchy skull fractures, patchy subdular hemorrhage, patchy subarachnoid hemorrhage, extensive damage to the brain tissues and a distorted, copper jacketed and medium caliber bullet is lodged and recovered inside the frontal lobe. GUNSHOT WOUND B: This is 3/16 inch close range penetrating wound in the cervical spine, at 8 inches below the top of the head, right along the posterior midline. There is a zone of soot around the entrance wound which measures 2 inch x 1inch in its greatest dimension. A hemorrhagic wound track es from postanterior and slightly upward causing severe damage to the cervical spine and the base of the brain tissue and a distorted, copper-jacketed and medium caliber bullet is lodged and recovered inside the sinus.
INTERNAL EXAMINATION The organs occupy normal positions and all the internal organs are in the state of autolysis. There are no adhesions or mass lesions. With the exception of the injuries stated above, all other part of the victims system appeared to be normal.
SUMMARY OF THE CASE: This 37 year old asian male was found at __(scene of the crime)__ with multiple gunshot wounds to his posterior head on __(date)__. EMS responded to the scene to confirm asystole. An autopsy was performed on __(date)__. Major autopsy findings include a total of two close gunshot wounds to the head with one close range penetrating gunshot wound on the right side of posterior head and one close range penetrating gunshot wound on the posterior cervical spine, with severe injuries to the brain and its surrounding areas. Based upon the circumstances surrounding the death, as currently known, the cause of death is multiple gunshot wounds to the head. The manner of death is murder.
